Guidelines and Terms

DIAKADI and Eating Free are looking for obese residents of the San Francisco Bay Area who have a desire to lose weight and embrace a healthier lifestyle in 2012.

We want to show individuals that living a healthy, fit and energized life is very possible. Our 2009, 2010, and 2011 Commit to Fit program was a huge success and our winners, Kevin Johnson, Kelley Clements and Dave McKew are still losing weight. Three years later, Kevin has lost over 160 pounds, changed his lifestyle and has truly found himself. Kelley has lost 52 pounds, increased her strength, and boosted her confidence. Still in year one, Dave has lost over 90 pounds and still saying “I can do this!” NOW is your opportunity to do the same!

Our nutrition, fitness, and life coaching will focus on reconnecting you with your body and introducing holistic approaches to monitor sleep and stress, identify eating strategies and establish goals.

The selected winner of the Commit to Fit Contest will share his/her progress with DIAKADI and Eating Free through website tracking, blogging, media outlets, and photos.

HOW IT WORKS
There is no cost to enter the contest. One winner will be selected after going through an application process in which final applicants may be interviewed and applicants’ written and video submissions will be reviewed by the DIAKADI and Eating Free teams. Entries can be submitted through Monday, January 31, 2012.

The winner will receive one free year of fitness and nutrition (a value of $15,000) starting the week of February 21, 2012. The Commit to Fit program includes the following:

  1. An in-depth assessment
  2. Three (3) personal training sessions a week from Mike Clausen, co-owner of DIAKADI
  3. Nutritional coaching from renowned Bay Area registered dietitian Sarah Koszyk which includes: Weekly Check-Ins for the First Four (4) Session. Following the four sessions, coaching takes place every other week. Online Food Journaling with Eating Free Nutrition Software
  4. Open Gym Membership at DIAKADI

ELGIBILTY
In order to be eligible for the Commit to Fit Contest, the Applicant must have a true desire to transform his/her life with fitness, weight loss, nutrition and holistic and wellness principles.

  • Applicant must be at least 18 years old.
  • Applicant must qualify as obese with a BMI greater than or equal to 30.
    To Calculate: BMI = (weight in pounds / (height in inches x height in inches) ) x 703
    Example of 200 lb, 5’5” individual: BMI = ( 200 / (65 x 65) ) x 703 = 33.3
  • Applicant must be available to meet for training and nutrition sessions Monday – Thursday between the hours of 9am – 5pm for 30 to 60 minutes.
  • Applicant must have the ability to commute to DIAKADI (SoMa, San Francisco) and Eating Free (Union Square, San Francisco) for training sessions.
  • Applicant must complete every step of the application process (see below).
  • Applicant must comply with all Commit to Fit program requirements for a full year.
  • Applicant cannot be a current or previous client of DIAKADI Body or Eating Free.
  • Applicant must be willing to sign all necessary liability waivers, medical record releases and agreements relating to participation in the Commit to Fit program.

Commit to Fit Personal Intake Form
Three-Minute Video (DVD, CD-ROM, Quicktime)*
DIAKADI Body Liability Waiver (Winner Only)
Eating Free Nutrition Liability Waiver (Winner Only)
Doctor’s Release Form (Winner Only)

*Video must include the following content:
Full Name
Age
Weight
Explanation of how a total body transformation would change Applicant’s life.

SELECTING THE WINNER
Selection of the winner will be based on the Applicant meeting the eligibility requirements identified above as well as on personal interviews of finalists. Selection of the winner will be within the sole discretion of the DIAKADI and Eating Free teams.

The Commit to Fit Contest will review the first 500 entries and narrow the selection down to five finalists.

The five finalists will then proceed to one or more in-person interviews. During this stage, Applicants may be asked to submit additional written materials, physician authorizations and authorizations for release of medical records.

The winner will be contacted on Monday, February 13, 2012. The Commit to Fit program will begin the following week!
